It is an honor to host the 9th Asia-Pacific Knee Osteotomy Symposium (APKOS) in Kanazawa, one of Japan's most representative traditional cities, with numerous distinguished guests. This symposium was held as the Korea-Japan Knee Osteotomy Symposium from its first to seventh symposium, but was reborn as APKOS last year due to the increasing prevalence of around-knee osteotomy (AKO) in the Asia-Pacific region.
Now, AKO in this region has developed uniquely, distinct from that of Western countries. This is likely due to factors such as the longevity of societies, lifestyles requiring deep knee flexion, and unique regional bone morphologies. Therefore, this symposium will be structured around the same theme as when I held the 6th Japan Knee Osteotomy Forum in 2017: "PHILOSOPHY OF OSTEOTOMY." Considering the aforementioned regional characteristics, while Japan has certainly become a long-lived society, healthy life expectancy is said to be more than 10 years shorter than the average life expectancy. Therefore, we aim to create a society where people can live long and healthy lives by obtaining knees that allow them to walk freely through AKO. Furthermore, while the remarkable recent advances in TKA and UKA have expanded the application of replacement arthroplasty to younger patients, even considering that they still fall short of AKO in deep flexion, there is still a strong indication for AKO in this region. Moreover, it has been reported that the medial tibial slope is larger in Asia, and the greater extra-articular deformity compared to Western countries suggests that the limitations of intra-articular correction with TKA and UKA differ, making AKO treatment strategies tailored to bone morphology even more crucial. Regarding surgical techniques, I would like to focus on safe and reliable procedures, which is a belief and philosophy that I have cherished for the 23 years since I became involved with AKO.
For these reasons, this symposium aims not only to discuss surgical techniques but also to approach the topic from a philosophical perspective: how to enrich people's lives. I conclude my remarks by expressing my hope that this symposium will serve as a significant opportunity to reconsider the indications, surgical procedures, and goal setting for AKO in the Asia-Pacific area.
